Firearms Licence Medical Proforma
As part of a firearms licence application, the applicant must arrange for their GP or another suitably qualified GMC-registered doctor to provide medical information to the police concerning their suitability to possess a firearm and/or shotgun.
Our GPs can complete the medical forms for a firearms licence. The cost for this service is:

You will need to provide:

Your full medical history from your current GP. A summary of your medical record is not adequate.

Your completed application form (with all sections completed except for the section entitled: “Medical Information: To be completed by doctor”). Most forms are available on the government website here.

Your consent for us to inform your NHS GP about your application as they must flag it on your NHS medical record.
