Testosterone Replacement Therapy (TRT)
Hormone treatment for men - led by experienced GPs
Our doctor-led Testosterone Replacement Therapy (TRT) service provides thorough assessment, diagnosis, treatment and ongoing monitoring.
Almost half of UK men have symptoms associated with testosterone deficiency, yet fewer than 5% receive a formal diagnosis*. Many men do not realise that their symptoms such as tiredness, low energy or reduced libido could be caused by low testosterone.
Testosterone is an important hormone involved in sexual function, muscle mass, bone health, energy, mood and general wellbeing. For men with persistently low testosterone together with appropriate symptoms, TRT could restore testosterone levels towards the normal range and relieve symptoms.

Symptom Check
The diagnosis of low testosterone and the decision to treat with TRT is based on both symptoms and low testosterone blood test results. The chart below helps you assess whether your symptoms are suggestive of low testosterone.
Q1: Do you have a decrease in libido (sex drive) and/or are your erections less strong?
YES - This may be caused by low testosterone. The next step would be to book a TRT Assessment.
NO - Go to Q2.
Q2: Do you have at least THREE of these symptoms?
-
Lack of energy
-
Decrease in strength or endurance
-
Loss of height
-
Decreased "enjoyment of life"
-
Feeling sad or grumpy
-
Recent deterioration in your ability to play sports or perform physical activity
-
Falling asleep after dinner
-
Recent deterioration in your work performance or concentration
YES - This may be caused by low testosterone. The next step would be to book a TRT Assessment.
NO - Your current symptoms do not match the typical pattern for low testosterone. If you would like to discuss your symptoms, we recommend booking a GP consultation.
TRT at The Maxwell Practice
The Testosterone Replacement Therapy (TRT) plan is a step by step assessment, treatment, and monitoring pathway designed to safely and appropriately treat men suffering from symptoms associated with low testosterone.
Our approach follows a clear principle: we assess first, and only proceed to treatment if it is medically appropriate for you. Low testosterone can cause symptoms such as low energy, low mood, reduced libido, erectile difficulties, loss of muscle mass and poor concentration - but these symptoms can also be caused by other treatable medical conditions (for example thyroid problems, vitamin deficiencies, diabetes, or depression). That is why our assessment is designed to identify the true cause of your symptoms.
1. TRT Assessment
A 30-minute appointment with a doctor, including a comprehensive panel of blood tests. We assess your testosterone and other hormones, look for potential medical causes of your symptoms and check that there are no obvious reasons why testosterone treatment would be unsuitable.
This appointment should be before 11am (because testosterone levels tend to fall throughout the day), and you should avoid sexual activity and vigorous physical activity for 48 hours beforehand.
Before booking the assessment, please review the Symptom Check above.
£375
2. Confirmation blood test
If the assessment blood test result indicates that you have low testosterone, this diagnosis needs confirming with a second blood test. This second test is required because testosterone levels naturally fluctuate, and we need confirmation that your levels are persistently low before starting treatment.
This appointment should be before 11am (because testosterone levels tend to fall throughout the day), and you should avoid sexual activity and vigorous physical activity for 48 hours beforehand.
£200
3. Treatment
We offer two forms of testosterone replacement medication - long-lasting injections or daily gel.
4. Monitoring
It is important that you are reviewed regularly and have monitoring blood tests. Our monitoring schedule is based on the BSSM approach with blood test undertaken at 3–6 months, at 12 months and annually thereafter.
You monitoring blood tests will check your testosterone levels as well as key underlying health checks such as PSA and haematocrit.

FAQs
What is TRT?
Testosterone Replacement Therapy (TRT) is a treatment used to restore testosterone levels in men who have testosterone deficiency and symptoms consistent with low testosterone.
Testosterone is an important hormone involved in sexual function, muscle mass, bone health, energy, mood and general wellbeing. When a man has persistently low testosterone together with appropriate symptoms, testosterone replacement may improve some of these symptoms and restore testosterone levels towards the normal range.
TRT is sometimes also referred to as testosterone therapy, hormone replacement therapy or hormone therapy for men.
What causes testosterone deficiency?
Whilst testosterone deficiency can be caused by factors such as injury to the testicles, testicular cancer, weight gain or a problem with the pituitary gland, the main reason for decline is simply age – it is natural for testosterone to decline as men get older.
For some men, this decline leads to a deficiency and causes unwanted symptoms. The name for the condition where you don't produce enough testosterone is hypogonadism.
What is a low testosterone level?
Testosterone is measured in nmol/L. In men, a level below 8 nmol/L is generally considered low, while levels between 8–12 nmol/L may also indicate testosterone deficiency, depending on the individual.
However, your testosterone level is only part of the picture. Men can experience symptoms at different testosterone levels, so your doctor will consider both your blood test result and your symptoms. A level that causes noticeable problems for one man may cause few or no symptoms in another.
Why must I have monitoring blood tests?
Monitoring allows your doctor to assess your response to treatment, check testosterone levels and identify potential side effects or changes that may require the dose or treatment approach to be adjusted.
We monitor important markers including testosterone, haematocrit and PSA, alongside other relevant health measures.
I started TRT with a different healthcare provider, can I continue my treatment with you?
Yes. Email or call us to let us know your situation and we will confirm the next steps.
